At your doctor’s office or in your home, the Community Care Team (CCT) works with you and your family to identify barriers to good health, and provide you with the tools and support you need to improve your health.
The Community Care Team works with your provider to help find areas in your healthcare where you may need more support, working with you to help you meet your needs. Your team can help you avoid expensive Emergency Room visits and hospital stays by:
- Setting Personal Goals
- Teaching about Health
- Improving Self-Care Skills
- Taking Medications Safely
The Community Care Teams helps you solve problems that can keep you from getting the care you need, such as:
- Getting to your Appointments
- Family Issues
- Finances/Work Schedule
- Other Social Service Related Needs
