Winterport Community Health Center Earns Highest NCQA Recognition as a Patient-Centered Medical Home

Penobscot Community Health Care (PCHC) is pleased to announce that the providers and staff of Winterport Community Health Center, 775 North Main Street, Winterport, have achieved Level III Recognition as a Patient-Centered Medical Home (PCMH) by the National Committee for Quality Assurance (NCQA), the highest level awarded for NCQA Recognition.

The Patient Centered Medical Home (PCMH) is a model for delivering comprehensive, coordinated, patient-centered primary care that is compassionate, accessible, and family-centered. The concept of PCMH is that improved access to primary care improves patient and provider experience, quality, and outcomes, as well as lowers cost.

Winterport Community Health Center is staffed by Dr. David Rocker and Dr. Larry Smith who have provided care to the Winterport community for more than twenty years. The practice is open to patients Monday – Friday, 8:00 am – 5:00 pm offering family medical care and mental health services.
