Seaport Community Health Center Earns Highest NCQA Recognition as a Patient-Centered Medical Home
We are pleased to announce that the providers and staff of Seaport Community Health Center, 53 Schoodic Drive, Belfast, have again achieved the highest level of recognition, Level III, as a Patient-Centered Medical Home (PCMH) by the National Committee for Quality Assurance (NCQA). Seaport Community Health Center providers and staff first earned recognition from NCQA as a Level III Patient-Centered Medical Home in 2013.
The Patient Centered Medical Home (PCMH) is a model for delivering comprehensive, coordinated, patient-centered primary care that is compassionate, accessible, and family-centered. The concept of PCMH is that improved access to primary care improves patient and provider experience, quality, and outcomes, as well as lowers cost.
Ken Schmidt, MPA, PCHC President and CEO, said, “Seaport Community Health Center has long provided an integrated, patient-centered model of care for the people of Waldo County and we are pleased that their high quality care was again recognized by the National Committee for Quality Assurance.”
Seaport Community Health Center is pleased to offer expanded hours for patients Monday-Friday and on Saturday, and in addition to family medical care, offers Integrated Medicine, Geriatric Care, Care Management, Pharmacy, Physical Therapy, Psychiatry, and Mental Health Services. About PCHC & Seaport Community Health Center
Seaport Community Health Center has offered primary care for the whole family and entire community for nearly 30 years, and has been an early adopter and leader of the electronic medical record, integrated behavioral health, and practice innovation. While Seaport has kept pace with the latest information technology (IT) and practice innovations, its providers still attend patients in the nursing home, make home visits, and run support groups for the elderly and those with opioid addiction.
In 2013, SCHC joined Penobscot Community Health Care (PCHC) a national leader among Federally Qualified Health Centers who provides an expanding network of primary care practices in and beyond the Bangor area. PCHC was founded in 1997 to ensure access to comprehensive, integrated primary health care services for all to improve the health and wellbeing of patients and Maine communities. Sixteen practices and program service sites in the Bangor area, and in Belfast, Winterport, and Jackman, offer a wide range of services including family medicine, dental, pediatrics, geriatrics, mental health and substance abuse, specialty services, pharmacy and healthcare for the homeless.
