Community Care Partnership of Maine ACO Achieves Medicare Shared Savings
Community Care Partnership of Maine (CCPM), an Accountable Care Organization (ACO) comprised of 11 member health care organizations throughout Maine, is pleased to announce the achievement of shared savings in its first year of participation in the Medicare Shared Savings Program (MSSP). For the 2016 performance year, CCPM achieved $4.3 million in total savings for Medicare, with $2.1 million in shared savings returning to CCPM and its member organizations – all of which are federally qualified health centers or critical access and community hospitals who are collectively committed to improving the quality and effectiveness of health care delivery and reducing unnecessary healthcare costs. By meeting MSSP quality performance and reporting measures, a remarkable feat in its first year, CCPM was able to earn 100% of possible shared savings. CCPM and its member organizations will use the performance payment to invest in infrastructure and continued improvements in the delivery of quality patient care.

CCPM serves 11,370 Medicare beneficiaries and is required to meet certain quality and performance metrics in the delivery of care for those patients, including in areas of patient/caregiver experience, care coordination and patient safety, preventive health, and treatment of at-risk populations with chronic disease. In the areas of preventive care and the treatment of at-risk populations with chronic disease, CCPM performed better than the mean performance rate of all participating MSSP ACOs for 14 out of 17 metrics. In addition, CCPM performed well in patient/caregiver experience, which is assessed through random surveys of Medicare members for the 2016 performance year.
The performance results also revealed key opportunities for improvement in the area of care coordination, specifically for unplanned admissions for patients with chronic disease, as well as readmissions for Skilled Nursing Facilities (SNF) – an area that CCPM had identified in advance and proactively established a SNF Collaborative to address.

Doctors, hospitals and health care providers establish ACOs in order to work together to provide higher-quality coordinated care to their patients, while helping to slow health care cost growth. Community Care Partnership of Maine is currently one of 480 ACOs participating in the Medicare Shared Savings Program. Shared Savings Program ACOs serve 9 million beneficiaries. ACOs receive a portion of the Medicare savings generated from lowering the growth in health care costs as long as they also meet standards for high quality care.
