PCHC Publishes 2017 Annual Report; Hosts 20th Annual Meeting
PCHC is pleased to make our Annual Report available to our patients, friends, and stakeholders. This report documents our success in 2017 and highlights PCHC’s integrated model of primary care behind every patient.
Through our integrated care model, our hope and our mission is that each patient will be met where they are, will be listened to, will meaningfully connect with the healthcare system, and will receive care, advice, and counsel that feeds the whole person – body, mind, and spirit.

PCHC hosted its 20th Annual Meeting at the Hilton Garden Inn in Bangor on April 10. Attended by PCHC providers, staff, and board members as well as local healthcare and business leaders, the meeting celebrated PCHC’s integrated model of care behind every patient.
