Jackman Community Health Center Earns Highest NCQA Recognition as a Patient-Centered Medical Home

We are pleased to announce that the providers and staff of Jackman Community Health Center, 376 Main Street, Jackman, have achieved Level III Recognition as a Patient-Centered Medical Home (PCMH) by the National Committee for Quality Assurance (NCQA), the highest level awarded for NCQA Recognition.

The Patient Centered Medical Home (PCMH) is a model for delivering comprehensive, coordinated, patient-centered primary care that is compassionate, accessible, and family-centered. The concept of PCMH is that improved access to primary care improves patient and provider experience, quality, and outcomes, as well as lowers cost.

Staffed by Patricia Doyle, MD, and Anya Babiarz, PA-C, the practice is open 8am-5pm Monday-Friday, with extended hours until 7pm on Thursday, and also offers 24/7 urgent care.  Jackman Community Health Center provides health care for over 1,000 patients with almost 4,000 patient visits each year and is the only primary care provider within 50 miles. The center offers family medicine, dental hygiene, pharmacy services, x-ray, 24/7 urgent care, mental health, and substance abuse services.
