Community Care Team (CCT)
CCTs are part of MaineCare’s Value‐Based Purchasing strategy, a multi‐pronged MaineCare initiative designed to improve patient experience of care, improve population health, and reduce costs.
CCTs are multi‐disciplinary, community‐based, practice integrated care management teams that deliver person‐centered health home services to MaineCare members with chronic conditions and complex care management and coordination needs. The model aims to improve healthcare outcomes while also reducing costs.

- Comprehensive Transitional Care
- Individual and Family Support
- Referral to community and support services
- Advanced Care Planning
- Culturally and Linguistically Appropriate Care
- Proactive Identification of Members who Need CCT Services
- Participation in Quality Improvement Initiatives
